Background: Pulmonary Aspergillosis is one of the outrageous impediments of erstwhile pulmonary diseases archetypally pneumonia or chronic obstructive pulmonary disease with 100% impermanence rate deprived of management. Formerly Aspergillosis has been reported to obfuscate austere Influenza globally. Nevertheless, Covid-19 Associated Pulmonary Aspergillosis (CAPA) is currently being reported in the ongoing Covid-19 pandemic.

Subject and method: A case study of a 57 years old immunocompetent male was admitted to the Department of Medicine at Allied hospital with severe respiratory distress. Covid-19 was inveterate through PCR (Polymerase Chain Reaction) for Severe Acute Respiratory Syndrome (SARS-COV 2) through nasal swab sampling. Another test was done named High-Resolution Computed Tomography (HRCT). The patient was managed by non-invasive ventilation as well as pharmacological therapy with Ivermectin, Methyl-Prednisolone, Remdesivir, Heparin, and broad-spectrum antibiotics along with symptomatic treatment.

 

Results: The result of PCR showed the presence of SARS-COV2 and result of HRCT was of chest exposed 80% lungs immersion with the appearance bilateral ground-glass opacities. Aspergillus fumigatus was cultured from sputum and the patient was diagnosed with Covid-19 Associated Pulmonary Aspergillosis (CAPA).

 

Conclusion: The  hypoxemia related symptoms of patient improved with the treatment and patient was getting better until day 16 when he underway worsening Regardless of the addition of Voriconazole in treatment practice, he died on 3rd day of diagnosis of CAPA due to severe respiratory failure.
