Fever, body-ache, malaise, retro orbital pain and petechia suggest a diagnosis of dengue fever in endemic areas. However, clinicians should also be vigilant about uncommon presentations and complications like dengue encephalitis, seizure, retinal edema, double vision, acute renal failure and pyelonephritis. Sometimes, a rapid card test like enterocheck can also be false positive due to cross reactivity. Abdominal pain is a red flag symptom and requires hospitalization. We have found 3 cases of hemorrhagic transformation of ovarian cyst as a potential differential diagnosis of unilateral abdominal pain in complicated dengue cases. Here we present a few atypical dengue fever cases in our institution.
