Ureteral Trauma Caused by Aberrant Foley Catheter Placement : A Case Report and a Review of the Literature

Abstract

Aberrant placement of urinary catheter into the ureter is a very rare but yet described complication and may cause complications.
We describe the case of a transurethral catheter misplacement into the ureter.
A 65-year-old women on long term indwelling urethral catheter exchange for neurogenic bladder. She complained of right flank pain and suprapubic discomfort consistent with low urine output 2 hours after she had her urine catheter changed. CT of abdomen and pelvis demonstrated an inflated foley catheter balloon in the proximal right ureter with right hydronephrosis and hydroureter. The aberrant placed catheter was removed and changed.
Delayed contrast-enhanced CT confirmed right proximal ureteral disruption with contrast extravasation into a contained collection in the right iliac fossa and foley catheter was seen into the bladder. She went well on discharge.
We also reviewed 28 cases that have been reported thus far in the literature to identify incidence, outcomes, risk factors and possible treatment to be taken.
The main risk factor identified is long-term catheterization in patients with neurogenic bladder. Major complication seems tobe ureteral disruption and can be managed either by conservative treatment or by urinary diversion. Surgical repair is indicated for severe ureteric injury. We should keep in mind the possibility of occurrence of this complication in such group of patients.Recognition of this condition is critical to institution of appropriate therapy and prevention of further complications.
