Innovative Approaches to Managing Intracanal Separated Instruments: The Bypassing and Braiding Techniques in Clinical Practice

Abstract

The management of separated endodontic instruments remains a complex and critical challenge in root canal treatment. When an instrument fractures within the root canal, it can obstruct access to the apical portion of the root, hindering effective debridement and disinfection. This can compromise the overall success of endodontic therapy, increasing the risk of persistent infection and treatment failure. The prognosis of cases involving separated instruments depends on multiple factors, including the location of the fragment, the size and type of the instrument, the degree of canal curvature, and the approach used for retrieval or bypassing. Recent advancements in endodontic techniques and technology have significantly improved the management of separated instruments, allowing clinicians to handle these situations with greater precision and predictability. However, conventional instrument removal techniques often require extensive dentin removal, which weakens the tooth structure and increases the risk of vertical root fracture. Therefore, selecting a conservative approach that preserves as much dentin as possible is crucial for maintaining long-term tooth integrity.
This presentation discusses a case series illustrating the management of separated instruments using bypassing and braiding techniques. The separated instruments in these cases were located at different levels within the middle and apical thirds of maxillary and mandibular incisors and canines. While instrument retrieval remains a viable option in many cases, it is not always the most suitable approach, particularly when the risk of excessive dentin loss outweighs the benefits of removal. Bypassing involves negotiating around the fractured instrument to establish a new glide path, allowing for continued cleaning, shaping, and obturation of the canal without removing the fragment. This technique minimizes dentin loss and preserves the original canal anatomy. Braiding, on the other hand, is a technique in which multiple fine instruments are intertwined around the fractured segment to facilitate its removal with minimal structural compromise.
Both techniques are considered conservative, simple, and effective alternatives to traditional removal methods, particularly in cases where complete instrument retrieval poses a high risk of weakening the tooth.
Through the discussion of clinical cases, this presentation aims to highlight the decision-making process in managing separated instruments, emphasizing the importance of case-specific evaluation, technique selection, and a minimally invasive approach. By integrating these conservative techniques into endodontic practice, clinicians can enhance treatment outcomes while preserving the structural integrity of the affected tooth.
